I am a few weeks in to Roamio ownership and about at the end of my rope. Roamio Plus, Cablevision, NDS cablecard. I can only go about 24 hours (sometimes 48) before I start getting V58 channel not authorized errors on different stations. Most likely you have good downstream but bad upstream so the card can't stay paired. I had this happen plenty of times. Had to go through the guided setup until it repeated the problem. Replace the cable card as previously recommended, It'll help you out the most. May have 1 bad tuner, check all of them to make sure!

Testing with channel 759. It sounds like it might be the same thing. Not a reception problem. Each time the problem occurs the cable system has sent a new/different PMT which the host is required to send to the CableCARD.
