Rule of moding number one: Never uninstall mods mid-playthrough the only exception is if the mod author himself says that you can. Due to pip-pad type mods physically removing the pip boy from your inventory, use console commands to add one and then equip it using player.additem and player.equipitem.

This happens mostly If you had pipboy mods (Pad Style) installed and didnt uninstall them correctly. Reinstall that mod and look at the moddescription how to uninstall them. it's possible a mod unequipped it from your character.
